About this product
FacilityGuard is a Snowflake Native App that surfaces resolved skilled-nursing-facility ownership and an ID-keyed change-of-ownership alert feed — with private-equity, REIT, and holding-company typing. It answers who owns a facility, how that ownership has changed over time, and which changes look like a genuine ownership event versus filing noise, each with a plain-language confidence note.
Because it installs from the Marketplace and runs in your account, your facility list and analysis never leave Snowflake. Ownership is refreshed monthly. CMS public data captures only part of PE investment activity; each alert states plainly what it can and cannot confirm.
Offered in tiers: a Vendor tier for self-serve network and data teams, and a Diligence tier for deal-window use with uncapped facility coverage. Enterprise terms available via private offer.
